Trust by Design: Why AI Fails When Meaning Is Unclear

Trust in AI does not happen by accident. It is built the same way trust in anything is built: by being deliberate and consistent. For AI, that starts with shared meaning.
"See you at the bank later?" a colleague asks.
"Of course," you reply. You know exactly what they mean.
For an AI or an outsider, what does this mean? At the river bank? At a financial bank? That seems odd.
They have no way to know that's your in-joke for the CFO's office.
People build shared meaning over years of lived experience. We absorb context through thousands of interactions until it becomes invisible. We stop noticing how much we carry.
AI does not build understanding the same way, but it does not arrive empty either. Large language models come with their own version of what words mean, learned from vast amounts of general text. That general knowledge is useful, but it can also be the source of the problem. When your business uses a word differently, AI will default to the general meaning unless you tell it otherwise. It does not know what it does not know about your organisation.
The previous article in this series introduced the idea of a semantic foundation, the layer of shared meaning that sits alongside your data model and metrics layer. Without it, AI can return the right number described in the wrong language, answer a different question to the one that was asked, or lose trust before it ever delivers value.
That article explained why context matters . Today we look at why designing trust in AI needs to be intentional, not something that you hope comes along later.
What happens when meaning is implicit in AI
Consider a few scenarios that go beyond a chatbot giving a wrong answer.
- An AI system summarises a lengthy compliance report for a board pack. The source document distinguishes between "material risk" and "operational risk." The AI treats them as interchangeable. The summary reaches the board with that distinction collapsed. No one catches it because the summary reads well, and the decision that follows is based on a blurred picture.
- A triage system classifies inbound customer cases, where"complaint" and "dispute" mean different things in your business. One triggers a standard response, and the other triggers a regulatory obligation. The AI doesn’t know the distinction. It classifies based on general language patterns. Cases get misrouted, some of which carry regulatory exposure.
There are points where AI filling the gap with general knowledge is acceptable, and there are also times it is a major failure. Knowing which cases need specific business context is now part of the work.
To be clear, not all meaning is going to be necessary. You will get along fine without it understanding the context behind "The Bank".
What designed trust in AI looks like
Trust by design means you do not wait for an AI failure to discover a gap in meaning. You surface those gaps before they become incidents.
In practice, that means three things working together.
Making meaning explicit. This is the starting point. Identify the terms your organisation uses where the internal meaning differs from the general meaning. Document the synonyms, the relationships, and the boundaries. Where two teams use different words for the same concept, make that visible. Where one word means different things in different divisions, define each usage and its scope.
This is what a semantic foundation captures. How metrics are calculated as well as what the language around those metrics mean to you. It works alongside your data model and your metrics layer.
The more explicit you make your context, the less wiggle room AI has to fill in the blanks with its own assumptions. That means more consistent outputs. AI does not always give the same answer to the same question. The less context it has, the more room it has to vary. Narrowing the range of possible interpretations makes outputs more predictable and more trustworthy.
Semantic systems are built in layers. It starts with vocabulary control: agreeing on the terms and their definitions. From there, you structure those terms into hierarchies and relationships, then layer in richer context and rules that describe how concepts relate to each other. Each layer prepares for the next. You do not need to build the whole thing before you see value. A clean, governed vocabulary is already a meaningful step forward.
Governing how meaning evolves. Meaning is not static. New products or business lines create new terms. Acquisitions bring conflicting definitions. Regulatory changes shift what matters. A semantic foundation that is built once and left alone will drift out of alignment with the business it serves.
This is a governance challenge, not a documentation project. It needs ownership, regular review, and versioning. The same disciplines you apply to your data assets apply here. If no one is accountable for the accuracy of your business language, your AI will eventually start using stale or incorrect context. That erodes trust just as quickly as having no context at all.
Keeping people central. Designed trust does not mean removing humans from the process. It means giving them better tools to verify what AI is doing and why.
When AI uses language that matches how the business actually thinks, people can assess the output quickly. They can see whether the answer makes sense in their context. They can spot when something is off. That is the difference between AI that people use with confidence and AI that people quietly stop using.
The goal is informed trust. People who understand what AI knows, what it does not know, and where the boundaries are.
Data governance teams have been doing this work for years. This is why data governance is the natural on-ramp to AI trust. The principles are identical: define what matters, assign accountability, manage change over time, and make sure everyone is working from the same shared understanding. The shift is extending the idea of "everyone" to include your AI systems alongside your people.
Onboarding AI is not that different to onboarding people
Think about what happens when a new person joins your organisation. You teach them the language. You explain the definitions, the processes, and the unwritten rules. You give them context that helps them understand how the business actually works.
Onboarding AI requires the same thing. The difference is that a person can ask clarifying questions, pick up on cues, and absorb context over months. AI cannot. It needs that context upfront and in a structured form.
The good news is that this is not two separate efforts. The work you do to make your business language explicit for AI also makes it clearer for people. New starters get access to governed definitions, clear hierarchies, and documented relationships between concepts instead of learning by osmosis over six months. Experienced staff get a shared reference point instead of relying on assumptions that may have drifted between teams.
Any investment you make in structuring your business context should serve both audiences. If it only works for machines, it is too technical. If it only works for humans, it is not scalable. The sweet spot is context that is clear enough for a person to read and structured enough for a system to use.
You do not need to solve everything at once
Start with the terms your people already know are ambiguous. The ones where misunderstandings already happen between teams, even without AI involved. Those are your highest-value starting points because they represent real risk, not theoretical risk.
From there, build outward. Each term you make explicit is one fewer assumption your AI has to guess at. The more context you provide, the more AI can do safely. The less context it has, the more likely it is to get the language wrong, lose trust, and stall adoption.
We spend our entire lives building understanding of language through lived experience. Your AI does not have that time. You have to build the context for it deliberately.
The goal is AI that your people trust enough to use, and your leaders trust enough to act on.
That trust is designed. It does not arrive by default.
